Simply mail in proof of purchase and Gibson will refund: $25 on product valued between $100 and $399 $50 on product valued between $400 and $799 $75 on product valued at more than $800 You must provide a copy of your invoice showing the product purchased price, the original UPC barcode label from the Gibson package, this form completely filled out and postmark it no later than January 31, 2015.
